Compiled and maintained inpatient medical records across beds, including admission notes, daily progress notes, discharge summaries and records. Organized serial laboratory results, imaging and pathology reports into structured timelines for case discussions and specialist consultations. Condensed each patient's entire course into a handover summary another clinician could act on immediately, under time pressure and without prior context. Reconciled records produced by different departments, resolving mismatches in dates, units, test naming and patient identifiers before they reached a decision point.
